FAMILY INCOME INQUIRY FORM
Why are you asked to fill out the Family Income Inquiry Form?
A while back, the Family Income Inquiry Form was used to decide which families in NYC needed financial assistance with food for their school-aged children. In 2017, all breakfast and lunch meals became free for every student in the New York City Public Schools system. Since then, this form is now used to determine the types of specialized funding schools receive based on the income information families provide. By completing this form, you give us a better chance of funding for needed staff, technology, and other resources. You should fill out the form even if you think your income is too high to qualify for benefits. You can always check “I do not want to share my information” if you prefer not to share your income level. Since the benefits are awarded based on the demographics and participation, filling out the form is always better than not filling out the form.
While there is no set due date yet, “the sooner the better” is the best plan. Therefore, if you haven’t completed the FamilyIncome Inquiry Form yet, please go to myschoolapps.com and spend approximately 7-10 minutes per FAMILY to complete the form. If you don’t know your child’s OSIS #, please contact Ricky Hutchins at rhutchins@ps130brooklyn.com
NOTE: This is a completely confidential form. P.S. 130 only has access to the cumulative data of the form, not individual access. Also, as of February 2nd, we have had approximately 62% of families complete this form. Our target is 90%.
